Please complete the food and beverage chart below.
Please write down everything you eat and drink for the next 3 days. It is important for you to write down the time when you consume anything. I promise I’m not judging what you eat or going to suggest some sort of extreme diet to get you sleeping well. I am only looking for things that may be influencing how you sleep. Most of it will have to do with the timing of the food or beverages you consume.
